Please wear short sleeves. No pre-registration required.

 

IMPORTANT INFORMATION ON INSURANCE & FEES

Cost covered under the following insurances:
Medicare Part B, Blue Cross/Blue Shield (all states), ConnectiCare and Aetna. Bring Medicare or insurance card with you for verification.  If your insurance type is not listed, you may be able to seek reimbursement directly from your insurance after paying for the immunization out of pocket; we will have forms available.

All others pay: Flu: $40.00 | Pneumonia: $60.00 | Both: $70.00

If paying cash, please bring exact amount. Make checks payable to Day Kimball Healthcare.
