Flu and pneumonia immunization clinic for adults, open to the public. 

The cost of the immunizations is covered under Medicare Part B, Blue Cross/Blue Shield (all states), Connecticare and Aetna. Self-pay is $40 for flu immunization, $60 for pneumonia immunization, or $70 for both.

Please bring your Medicare or insurance card with you for verification. If your insurance type is not listed, you may be able to seek reimbursement directly from your insurance after paying for the immunization out of pocket (forms are available).

If paying with cash, please bring the exact amount. Checks may be made payable to Day Kimball Healthcare. 

A provided permission form must be completed prior to vaccination. Please wear short sleeves.
