One recent development in health care is the establishment of a “patient centered medical home” (PCMH) model of care within integrated health care systems. In this model, your primary care practitioner is at the core of your care and the entire care team at your primary care office is your PCMH.As your PCMH, your primary care team works together to communicate closely across the entire network of services to proactively help you access the care you need. This helps eliminate errors and redundant services, which in turn controls costs and assures you of top-quality care and advice.
